General Terms and Conditions (GTC) and Privacy Policy

1. drug-explorer.com provides database based scientific information for doctors and is not a tool for self-diagnosis or self-treatment of medical non-experts. drug-explorer.com cannot assume any liability for possible unintended side-effects, damage or other disadvantages that could be attributed to recommendations of drugexplorer.com. drugexplorer.com is not a substitute for consultation of a specialist. As indicated at the end of any results table, any medical decision hast to be taken by a qualified and experienced physician, who is responsible for this decision.

Medicine is not an exact science. On the one hand, there is not a sufficiently well-founded and scientifically valid opinion for every question, and on the other hand recommendations and opinions may differ depending on the scientific source. Not all drugs are approved in all countries. Even though a drug is approved, indications, contraindications and recommendations for dosing may differ between countries. Primarily, the given data refer to european and/or american recommendations. Data have been collected to the best of our knowledge.
drug-explorer.com is unable to assume any liability for the correctness or completeness or up-to-dateness of given data. In particular, it can not be excluded that approvals or recommendations or other factors may differ or change depending on countries or continents.

2. General starting dose and General maximum dose refer to normal weighted adults without any restrictions.

3. If different recommendations for dosing in case of impaired renal or liver function are displayed, the right dosage has always to be chosen in consideratin of the general situation. In most cases, this will be the lower dosage.

4. Allergies to medications are not listed in the contraindications section since this refers to any medication and is logical.

5. In regards to drug-administration during pregnancy and/or lactation period the recommendations oft he american food and drug administration (FDA) are listed, as far as available. As a general point, it should be noted that there is no drug with a proven harmlessness for pregnancy and/or lactation period in several large controlled studies. Relative dose and milk-plasma-ratio are not available for all drugs. If selection is performed within these criteria, only those drugs will be returned for which respective values are present.

6. The selection options regarding the side effects, that should not be present, should be used very carefully and, of course, in consideration of the frequency of the side effects. If a side effect occurs very rare and is selected for not being present, this may cause the exclusion of medication that could be useful.

7. Activity data of antiinfectives refer to the current literature. However, regional or individual variations and resistances may deviate from this. The resulting antiinfectives, that are shown after selection of pathogens, are not automatically suitable for monotherapy of the infection, but show a probable susceptibility. Furthermore, account needs to be taken of the fact that besides from susceptibility tissue penetration plays an important role. Any antiinfective is ineffective, when it does no reach the site of infection. In case of doubt or in severe cases the consultation of a specialist for infectious diseases is recommended.

8. Chosen selection options must have a meaningful relationship. Therefore, it makes no sense to chose e.g. "pneumonia" as indication and "intraocular" as mode of administration.

9. In General contraindications and Absolute contraindications all entered contraindications are listed. In Relative contraindiactions only those contraindications are listed that are explicitly classified as relative contraindications. This means, that selection of a relative contraindication does not exclude that this contraindication may be absolute. In contrast to the side effects, there are no unclassified (i.e. neither relative nor absolute) contraindications.
With regard to the contraindications, that are displayed in a drug’s profile, any single contraindication has to be taken into account, even, if there is an overlap. When e.g. there are the contraindications "Children" and "Children below 8 years", children above 8 years have to be considered as contraindication, too, since they belong to the contraindication "Children". The occasional overlap of contraindications is due to the structure of the database.

10. Side effects are classified according to the frequency of occurence. Side effects without given frequency of occurence are listed in General Side effects where all other side effects are listed, too. This means that selection of a side effect in category General side effects may lead to the exclusion of drugs, that cause the selected side effect very rare, however, no exact frequency of occurence was reported.

11. When Cytochrom P450 options are selected only those substrates, inhibitors and inducers are processed that are clinically relevant. E.g. citalopram is a very weak inhibitor of CYP2C9, CYP2E1 and CYP3A4. Due to a lack of clinical relevance these parameters are not taken into account.

12. Data shown in the results table apply to the respective column, only. If there are several columns no cross-linking is possible between the data of one line (e.g. drug dosing in liver cirrhosis, renal failure and children). In most cases the lowest dosage of the provided dosages will be the right one.

13. In general, mild hepatic impairment is to be understood as Child A cirrhosis, moderate hepatic impairment as Child B cirrhosis and severe hepatic impairment as Child C cirrhosis. However, hepatic impairment is not always linked to cirrhosis (e.g. acute liver failure, alcoholic steatohepatitis). It is clear that hepatic impairment has to be taken into account in these cases in regards to drug selection and dosing, too.

14. If there are several ways for administration of a drug, it is possible that a certain contraindication applies for one mode of administration but not for another. For example, liver cirrhosis is a contraindication for oral administration of budesonide, however, not for inhalative administration. Since contraindications in the database are drug-specific and not administration specific, the selection of a contraindication leads to the exclusion of a drug in all modes of administration. For example, selection of liver cirrhosis as "contraindication that should not be present" would lead to the exclusion of e.g. budesonide in all modes of administration.

15. Frequencies of side effects may vary with different dosages. For example, bleeding complications will be more frequent when a drug like enoxaparin is used with a higer dose.

16. If additional comments are shown in the line of a drug in the results table (e.g. when indications were selected), these comments are only usable when only 1 parameter (i.e. e.g. only 1 indication) was selected.

17. If cardiotoxicity is selected as side effect that should not be present, all drugs that may cause QT prolongation will be excluded as well. If QT prolongation is selected as side effect that should not be present, only those drugs will be excluded for which QT prolongation is a potential side effect. However, drugs that may cause cardiotoxicity beyond QT prolongation will not be excluded.

18. If a drug profile is selected, indications, absolute and relative contraindications are shown. However, not shown are possible comments and restrictions. Any user of a drug is responsible for the right and indication-appropriate use of the drug.

19. drug-explorer.com is not perfect since medicine is not perfect. Different data from different sources may vary or even be contradictory. Apart from that, different people may show differing reactions to one and the same drug. Despite all this limitations, drug-explorer.com is superior to any doctor with regard to multi-criteria drug selection.
